Passport services are
                conducted on a walk-in basis, no appointment necessary.

   Our office is conveniently located on the east side of Meridian,
across from Pioneer Park and the Puyallup Public Library. 

What do you need to apply for a passport or passport card?

  • Completed DS-11 Passport Application (Revised 09-2013)
  • 1 recent color Passport Photo (local drug store, photography studio, or other locations)                                    
  • Proof of Citizenship
  • Identification
  • Fees 
    • A $25 passport execution fee, in addition to the passport application fees, is charged to applicants applying on Form DS-11. 
    • Acceptable forms of payment: personal/business checks, money orders or cashier's checks.

  • Each passport applicant (including children) must apply in person
  • You must use Black Ink only when filling out your application

What do you need to renew a minor's (15 years of age and under) passport or passport card?

  • A Completed DS-11 Passport Application (Revision date 09-2013)
  • 1 recent color Passport Photo (from a local drug store, a photography studio, or a variety of other locations)                                           
  • Proof of Citizenship - Original or copy of their certified birth certificate AND previous passport or card                        
  • Parents Identification
  • Permission of both parents/legal guardians either both in person or one in person with a notarized DS-3053 form of the non-present parent. A front and back copy of the parents ID must accompany the DS-3053 form.
  • Each child must appear in person at the time of application
  • What do you need to renew an adult passport or passport card?
  • A Completed DS-82 Passport Renewal By Mail Application                                                    
  • 1 recent color Passport Photo (from a local drug store, a photography studio, or a variety of other locations)                                             
  • Your most recent Passport or Card
  • A personal check, money order or cashier's check made out to the U.S. Dept. of State

PLEASE NOTE: ADULT PASSPORT RENEWALS ARE A MAIL-IN PROCESS ONLY.  The city acceptance agents will, as a courtesy, review your application prior to you mailing it.
