Passport Services are available
Monday through Friday -
8:30 a.m. – Noon

Wednesday afternoons - 1:00 p.m. to 4:00 p.m.
(Except Holidays)

Our office is conveniently located on the east side of Meridian,
across from Pioneer Park and the Puyallup Public Library.  Passport services are 
conducted on a walk-in basis, no appointment is necessary.

 

Beginning April 1, 2011, the U.S. Department of State will require the full names of the applicant’s parent(s) to be listed on all certified birth certificates to be considered as primary evidence of U.S. citizenship for all passport applicants, regardless of age.  Certified birth certificates missing this information will not be acceptable as evidence of citizenship.  This will not affect applications already in-process that have been submitted or accepted before the effective date.

What do you need to apply for a passport?
  • A Completed Passport Application (Revision date 12-2010)
  • 1 recent color Passport Photo
  • Proof of Citizenship
  • Identification
  • Fees -- Please note: the city is not authorized to accept credit/debit cards.
                Acceptable forms of payment include personal/business checks, money
                orders or cashier's checks.
  • Each passport applicant (including children) must apply in person
