To better serve the community, additional passport
processing hours are now available!
Wednesday afternoons - 1:00 p.m. to 4:00 p.m.
Our office is conveniently located on the east side of Meridian,
adjacent to the Puyallup Public Library. Passport services are
conducted on a walk-in basis, no appointment is necessary.
What do you need to apply for a passport?
- A Completed Passport Application
- 2 recent Passport Photos
- Proof of Citizenship
- Identification
- Fees
Online Passport Forms
- Application for a new passport (DS-11)
- Application for renewal by mail (DS-82)
- Re-application form (DS-5504) -- Changes/Corrections to current passports
- Statement regarding lost/stolen passport (DS-64)
- Statement of parental consent for applicants under age 16 (DS-3053)
- Application for additional visa pages (DS-4085)
