Passport Services are available
Monday through Friday - 8:30 a.m. – Noon
Wednesday afternoons - 1:00 p.m. to 4:00 p.m.
(Except Holidays)
Our office is conveniently located on the east side of Meridian,
across from Pioneer Park and the Puyallup Public Library. Passport services are
conducted on a walk-in basis, no appointment is necessary.
What do you need to apply for a passport or passport card?
- A Completed DS-11 Passport Application (Revision date 12-2010)
- 1 recent color Passport Photo
- Proof of Citizenship
- Identification
- Fees -- Please note: the city CANNOT accept credit/debit cards.
Acceptable forms of payment include personal/business checks, money
orders or cashier's checks.
- Each passport applicant (including children) must apply in person
What do you need to renew a minor's (15 years of age and under) passport or passport card?
- A Completed DS-11 Passport Application (Revision date 12-2010)
- 1 recent color Passport Photo
- Proof of Citizenship (birth certificate AND previous passport or card)
- Parents Identification
- Permission of both parents/legal guardians either both in person or one in person with a notarized DS-3053 form of the non-present parent. A front and back copy of the parents ID must accompany the DS-3053 form.
- Each child must appear in person at the time of application

- 1 recent color Passport Photo
- Your most recent Passport or Card
