Statement of Qualifications

Architectural and engineering consultants interested in notification and consideration for city facility projects must submit a statement of qualifications summary to the Parks and Recreation Department.  The information on this form will be used by the the City of Puyallup Parks and Recreation Department to assist in filing and retrieving the statements of qualifications submitted by architectural and engineering consultants.
